It began with a series of investigative articles that I wrote for the journal Science and then the New York Times Magazine on the surprisingly dismal state of nutrition and chronic-disease research. Its arguments were honed in lectures at medical schools, universities, and research institutions throughout the United States and Canada.

Why Were They Fat? 

eBookImagine you’re serving on a jury. The defendant is accused of some heinous crime. The prosecuting attorney has evidence that he says implicates the defendant beyond reasonable doubt. He says the evidence is as clear as day and that you must vote to convict. This criminal must be put beyond bars, you’re told, because he’s a threat to society.The defense attorney is arguing just as vehemently that the evidence is not so clear-cut. The defendant has an alibi, albeit not one that’s airtight. There are fingerprints at the crime scene that don’t match the defendant’s. He suggests the police may have mishandled the forensic evidence (the DNA and hair samples). The defense argues that the case is not nearly as definitive as the prosecutor has led you to believe. If you have reasonable doubt, as you should, you must acquit, he says. If you put an innocent man behind bars, you’re told, not only do you do that person an incalculable injustice, but you leave the guilty party free to strike again. 

A Primer on the Regulation of Fat 

PDFIt’s time to roll up our sleeves and get to work. What we need to know is what biological factors regulate the amount of fat in our fat tissue. And, specifically, how this is affected by our diets, so we can know what we’re doing wrong and how to change it. Another way to say this is that we need to know what determines nature—why we might be predisposed to get fat or stay lean—and what elements of nurture, of diet and lifestyle, can be altered to affect thispredisposition or combat it.I’m going to be discussing some basic biology and endocrinology, subjects you may understandably find slow going. All I can promise is that if you pay attention you’ll know virtually everything you need to know about why people get fat and what has to be done to combat it.

Why Diets Succeed and Fail 

eBookThe simple answer to the question of why we get fat is that carbohydrates make us so; protein and fat do not. But if this is the case, why do we all know people who have gone on low-fat diets and lost weight? Low-fat diets, after all, are relatively high in carbohydrates, so shouldn’t these fail for all the people who try them?Most of us know people who say they lost significant weight after joining Weight Watchers or Jenny Craig, after reading Skinny Bitch or French Women Don’t Get Fat, or following the very low-fat diet prescribed by Dean Ornish in Eat More, Weigh Less. When researchers test the effectiveness of diets in clinical trials, like the Stanford University A TO Z Trial that I’ll discuss shortly, they’ll invariably find that a few subjects do indeed lose considerable weight following low-fat diets.Doesn’t this mean that some of us get fat because we eat carbohydrates and get lean again when we don’t, but for others, avoiding fat is the answer?The simple answer is probably not. The more likely explanation is that any diet thatsucceeds does so because the dieter restricts fattening carbohydrates, whether by explicit instruction or not. To put it simply, those who lose fat on a diet do so because of what they are not eating—the fattening carbohydrates—not because of what they are eating.

Almost everything is, really.)But my mom believed I required treatment, so she took me to see my Regular Doctor Jim, who agreed that I was veritably swimming in a paralyzing and totally clinical depression, and that therefore my meds should be adjusted and also I should attend a weekly Support Group.This Support Group featured a rotating cast of characters in various states of tumor-driven unwellness. Why did the cast rotate? A side effect of dying. The Support Group, of course, was depressing as hell. It met every Wednesday in the basement of a stone-walled Episcopal church shaped like a cross. We all sat in a circle right in the middle of the cross, where the two boards would have met, where the heart of Jesus would have been.I noticed this because Patrick, the Support Group Leader and only person over eighteen in the room, talked about the heart of Jesus every freaking meeting, all about how we, as young cancer survivors, were sitting right in Christ’s very sacred heart and whatever.

CHAPTER TWO 

The Fault in Our StarsAugustus Waters drove horrifically. Whether stopping or starting, everything happened with a tremendous JOLT. I flew against the seat belt of his Toyota
SUV each time he braked, and my neck snapped backward each time he hit the gas. I might have been nervous—what with sitting in the car of a strange boy on the way to his house, keenly aware that my crap lungs complicate efforts to fend off unwanted advances—but his driving was so astonishingly poorthat I could think of nothing else.We’d gone perhaps a mile in jagged silence before Augustus said, “I failed the driving test three times.”“You don’t say.”He laughed, nodding. “Well, I can’t feel pressure in old Prosty, and I can’t get the hang of driving left-footed. My doctors say most amputees can drive with no problem, but . . . yeah. Not me. Anyway, I go in for my fourth driving test, and it goes about like this is going.” A half mile in front of us, a light turnedred. Augustus slammed on the brakes, tossing me into the triangular embrace of the seat belt. “Sorry. I swear to God I am trying to be gentle. Right, so anyway, at the end of the test, I totally thought I’d failed again, but the instructor was like, ‘Your driving is unpleasant, but it isn’t technically unsafe.’”“I’m not sure I agree,” I said. “I suspect Cancer Perk.” Cancer Perks are the little things cancer kids get that regular kids don’t: basketballs signed by sports heroes, free passes on late homework, unearned driver’s licenses, etc...

The Fault in Our StarsI stayed up pretty late that night reading The Price of Dawn. (Spoiler alert: The price of dawn is blood.) It wasn’t An Imperial Affliction, but the protagonist, Staff Sergeant Max Mayhem, was vaguely likable despite killing, by my count, no fewer than 118 individuals in 284 pages.So I got up late the next morning, a Thursday. Mom’s policy was never to wake me up, because one of the job requirements of Professional Sick Person is sleeping a lot, so I was kind of confused at first when I jolted awake with her hands on my shoulders.“It’s almost ten,” she said.“Sleep fights cancer,” I said. “I was up late reading.”“It must be some book,” she said as she knelt down next to the bed and unscrewed me from my large, rectangular oxygen concentrator, which I called Philip, because it just kind of looked like a Philip.Mom hooked me up to a portable tank and then reminded me I had class. “Did that boy give it to you?” she asked out of nowhere.“By it, do you mean herpes?”“You are too much,” Mom said. “The book, Hazel. I mean the book.”“Yeah, he gave me the book.”“I can tell you like him,” she said, eyebrows raised, as if this observation required some uniquely maternal instinct. I shrugged. “I told you Support Group would be worth your while.”“Did you just wait outside the entire time?”“Yes. I brought some paperwork. Anyway, time to face the day, young lady.”“Mom. Sleep. Cancer. Fighting.”“I know, love, but there is class to attend. Also, today is . . . ” The glee in Mom’s voice was evident.“Thursday?”“Did you seriously forget?”“Maybe?”“It’s Thursday, March twenty-ninth!” she basically screamed, a demented smile plastered to her face.“You are really excited about knowing the date!” I yelled back.“HAZEL! If you wish to stop at a particular point, you need an argument to do so.It might well be true that lowering a highway speed-limit from 70 mph to 60 mph would save lives. That is not a sufficient argument for choosing 60 mph, however, because lowering the speed-limit to 50 mph would save even more lives. And more still would be saved at 40 mph. 

The obvious conclusion of this runaway train is that if saving lives is the sole aim, the speed-limit should be set at the level which saves the most, and this is 0 mph.In practice the lives at risk for each proposed speed-limit have to be measured against what is achieved by the ability to travel and to transport goods rapidly. Most of our daily activities involve a degree of risk which could be reduced if we limited ouractions. In practice we trade off risks against convenience and comfort. If the case for making the speed-limit 60 mph is based solely on the lives which could be saved, the arguer will need additional reasons to stop at 60 mph before the runaway train of his own argument takes him to 50 mph, then 40 mph, andfinally crashes into the buffers when it reaches 0 mph.
